The 34-year-old mother of two did not enter a plea to the charge, and has been remanded in custody while psychiatric reports are prepared.
West was arrested after a video showing a woman racially abusing passengers on the number 3 tram from New Addington to Wimbledon was posted on YouTube.
The video, entitled My Tram Experience, has been viewed 2.2million times since it was posted on Sunday.
Police have now asked YouTube to remove the video.
Croydon Magistrates' Court heard today that West has been suffering from depression since she was 18, receiving treatment at facilities in Purley and West Croydon.
West, wearing a pink Adidas tracksuit top, appeared visibly distressed throughout the hearing and sobbed into her hands as she was remanded in custody while psychiatric reports are prepared.
She is due to appear before Croydon Magistrates' Court again on December 6.
